Participation Forms
Participating homeowners and contractors must each fill out a form for every location where a ductless heat pump is installed as part of the project.  The information on these forms determines home and homeowner eligibility, while supporting NW Ductless Heat Pump Project evaluation efforts. The Homeowner Participation and Contractor Invoice forms are available below as downloadable pdf files.

ATTENTION: SOME UTILITIES REQUIRE PRE-APPROVAL PRIOR TO DUCTLESS HEAT PUMP INSTALLATION.  PLEASE CHECK WITH THE UTILITIES IN YOUR SERVICE TERRITORY TO FIND OUT IF THEY HAVE SPECIFIC REQUIREMENTS.

THE UTILITIES LISTED BELOW HAVE UNIQUE PARTICIPATION FORMS THAT MUST BE USED FOR INSTALLATIONS IN THEIR AREA.
